Endoscopist Log Book

The Endoscopist Log Book provides a detailed list of procedures performed by an Endoscopist over a selected period.

Report Structure

Position	Column	Description
1	Endoscopist	Name of Endoscopist
2	Roles	Role in the procedure (e.g. Endoscopist, Trainee Endoscopist, Nurse Endoscopist)
3	Site Name	Site where the procedure was performed
4	Room Name	Room in which procedure was performed
5	Procedure Id	Procedure’s identifier
6	Procedure Type	Procedure performed (e.g. Colonoscopy)
7	Procedure Date	Date when procedure was performed
8	Referral Date	Recorded referral date for episode
9	List Type	Service (Planned) or Training List Type
10	Appointment Type	Identifies the type recorded for the patient from the referral (e.g. Routine, Urgent)
11	Referral Source	Source of referral (e.g. Day case, In-patient)
12	Patient Management	Identifies the classification of the patient from the referral (e.g. Day case, Inpatient)
13	Patient Category	Identifies the type of the patient from the referral (e.g. NHS, Private)
14	Referral Reasons	The selected referral reason (s) that the procedure was performed
15	Findings	Details of procedure findings recorded
16	Therapeutics	Details of Therapeutic procedures performed
17	Extent	Recorded extent for procedure e.g. Caecum
18	Diagnosis	Details of initial procedure diagnosis recorded
19	Post Pathology Diagnosis	Details of post pathology procedure diagnosis recorded
20	Follow Ups	Details of Follow ups recorded
21	Referring GP	Patient’s recorded referring GP
22	Referring Cons.	Patient’s recorded referring Consultant
23	Referring Cons. Speciality	Patient’s recorded referring Consultant’s speciality
24	Hospital Identifier	Local patient identifier (e.g. Hospital Number)
25	National Identifier	National patient identifier (e.g. NHS or CHI Number)
26	Patient Name	Patient’s full name
27	Gender	Patient’s gender
28	DoB	Patient’s date of birth
29	Age at Procedure	Patient’s Age on the date the procedure was performed
30	Postcode	Patient’s postcode

Report parameters

Position	Column	Description
1	Units	Select from ALL or selected individual sites (in case of multiple site organisations)
2	Procedure	Select from ALL or selected individual procedure performed (e.g. Colonoscopy, OGD)
3	From	Date from which the report will interrogate the data
4	To	Date to which the report will interrogate the data
5	Endoscopist	Selected Endoscopist*

*Endoscopist – For standard Endoscopist role permission, the individual Endoscopist can review their own data. For a user with Manager role permission, all Endoscopist data can be reviewed.
