Referral Reasons

Use this screen to record the pertinent Referral Reasons for the procedure. The options presented will be relevant to the procedure(s) being performed.

When a patient is selected from the Daily List panel, if consent has already been recorded, relevant for Nurse Led consent recording, the Endoscopist will now be presented with the completed Assessment screen to review or update information relating to High Risk Issues and Current Medication before the Referral Reasons screen.

Some options are just a option button, but others will have additional attributes that will be displayed when you check the box, for example:

In the example above you must provide a value for the type attribute (which is why it is shown in red)

Some options may have more than one attribute, in the example below you are provided with a text box to enter any other relevant information. The text box expands based on its contents.
