6	Procedure Date	Date when procedure was performed
7	Agent used	Identifies the Reversal Agent used
8	Dose	Identifies the does given of the Reversal Agent used
9	National Identifier	National patient identifier (e.g. NHS or CHI number)
10	Hospital Identifier	Local patient identifier (e.g. Hospital Number)
11	Patient Name	Patient’s name
12	Gender	Patient’s gender
13	Date of Birth	Patient’s date of birth
14	Age	Patient’s Age on the date the procedure was performed
