Report Structure

Position	Column	Description
1	Hospital Identifier	Local patient identifier (e.g. Hospital Number)
2	National Identifier	National patient identifier (e.g. NHS or CHI Number)
3	Full Name	Patient’s Full Name
4	DoB	Patient’s date of birth
5	Referral Date	Referral date recorded for episode
6	Patient Category	Identifies the type of patient (e.g. NHS/Day case/Routine)
7	Procedure	Identifies the procedure created for the patient’s referral
8	Site and Room	Site Name & room recorded for procedure
9	Appointment Date	Recorded appointment date
10	Cancellation Date	Recorded cancellation date
11	Further Action	Recorded action required when completing the cancelation process on EMS
12	Comments	Space on report to place comments when extracted to MS Excel
13	Cancelled by	Name of user completing cancellation process on EMS
14	Completed by	Name of user completing the cancellation process on EMS following review
15	Completed At	Recorded Date and time cancellation process was completed
